Upon initial referral from either your Treating Specialist, General Practitioner, Veteran’s Liaison Officer or other DVA provider, a Registered nurse would come out to your place of residence and conduct a comprehensive assessment which would include assessing your clinical needs, goals, and develop a care plan.
They will then pass on their assessment to the Absolute Home Care rostering officers, who will source out the relevant compatible staff that will be providing care that meet assessed clinical needs.
Our team liaises with other allied health professionals such as Occupational Therapists, Physiotherapists, Dieticians, Podiatrists, Speech Pathologists, Home Based Palliative Care, General Practitioners, Treating Specialists, Veteran’s Liaison Officers, etc. in order to provide comprehensive and compassionate care to our consumers.
Our Clinical team will be reviewing your care as per DVA Guidelines and as necessary for the individual consumer.
